Admission Criteria

The person has a documented terminal diagnosis with a limited life expectancy of six months or less if the illness follows its expected course and has been so informed by the primary physician.

The person is under the care of a physician who will continue at assume responsibility for medical care.

There is a an identifiable, willing, capable caregiver in the home, unless other appropriate arrangements are agreed to by the patient, the family and the Hospice team.
- The person must reside within the Hospice of Cumberland County service area. Exemptions include, inpatient at Cumberland Medical Center, visiting Hospice Patients, Nursing Home or Assisted Living residents of Cumberland County.
